Users are creating new Topics that don't have a Topic Prefix selected. I don't know how that is happening, given that I myself tested this.
The Topic above: ACPI file; was just created, and it doesn't have a Topic Prefix. This Topic is in the Channel I've designated for use by the MOD, and I have Topic Prefixes for use in this Channel.
Why is this happening?

There is something about the Registered Users usergroup (the usergroup that new registrants are in until they have made three posts, at which time they are promoted into the Established usergroup) that prevents this MOD from being triggered.
Members in this usergroup can create and post Topics without selecting a Topic Prefix. I just tested this.

Snowhog, is my account on your forum in Registered Users usergroup?

The mod does not check the usergroup. It just checks the current Channel Node ID against the configured Forum Node ID. From my initial investigation, I didn't find anything that would cause the issue. I will continue investigating.

I found the issue! The Registered User usergroup on your forum only has permission to create Text Posts and doesn't have permission to create other content types (e.g. Photo Galleries, Poll, Links, Videos and Events). Basically, the toolbar icons at the upper left corner of the editor are not available. The JS for the mod is inserted in that location. Since registered users don't have those permissions, the JS is not inserted and thus the code for the Prefix validation is not present for that usergroup.
I will update the mod to fix this issue.

A new user created their first post early this morning. It is in one of the two Forums that I have this MOD turned on for. The Channel he created the Thread in does not have a Topic Prefix.
I think I know what is causing this (but don't know why) and hopefully you can provide a fix.
Because we have been getting so many new registrations with IPs resolving to Russia, I have opted to have all new registrations who haven't confirmed their registrations; acknowledged the registration email; placed in the Users Awaiting Moderation usergroup.
Added:
The post appears to have been redirected from Help the New Guy to Kubuntu 25.04 Plucky Puffin. But that still doesn't account for why the post, originally created in a Channel that this MOD affects, was made without a Topic Prefix. The Linux Community has given me much.
